58 Noninvasive positive pressure ventilation in unplanned extubation  BACKGROUND: Unplanned extubation is quite common in ICU pts receiving MV. The present study aimed to investigate the effectiveness of NIV in pts with unplanned extubation. MATERIALS AND METHODS: A total of 15 pts (12 male, age: 57 +/- 24 years, APACHE II score: 19 +/- 7) monitored at the medical ICU during the year 2004 who developed unplanned extubation were included in the study. NPPV was tried in all of them following unplanned extubation. Indications for admission to the ICU were as follows: 9 pts pneumonia, 3 status epilepticus, 1 with gastrointestinal bleeding, 1 with cardiogenic pulmonary edema and 1 with diffuse alveolar bleeding. RESULTS: 11 pts (74%) were at the weaning period at the time of unplanned extubation. Among these 11 pts, NPPV was successful in 10 (91%) and only one (9%) was reintubated due to the failure of NPPV. The remaining 4 pts (26%) had pneumonia and none of them were at the weaning period at the time of extubation, but their requirement for MV was gradually decreasing. Unfortunately, an NPPV attempt for 6-8 h failed and these patients were reintubated. CONCLUSIONS: Pts with unplanned extubation before the weaning criteria are met should be intubated immediately. On the other hand, when extubation develops during the weaning period, NPPV may be an alternative. The present study was conducted with a small number of patients, and larger studies on the effectiveness of NPPV in unplanned extubation are warranted for firm conclusions. Ervuksel E. 59 Factors predicting successful noninvasive ventilation in acute lung injury  NIV has potential as an effective therapeutic method in patients with acute lung injury (ALI).The aims of this study were to determine factors predicting the need for endotracheal intubation in ALI patients treated with NIV, and to promote the selection of patients suitable for NIV.  Methods. Ι CU of the Nippon Medical School Hospital from 2000 - 2006 with a diagnosis of ALI, in whom NIV was initiated.  Results. A total of 47 patients with ALI received NIV, and 33 patients (70%) successfully avoided endotracheal intubation.Pts who required endotracheal intubation had a significantly higher APACHE II score and a signifi cantly higher Simplifi ed Acute Physiology Score (SAPS) II, and a significantly lower arterial pH. The respiratory rate decreased significantly within 1 h of starting NIV only in patients Successfully treated with NIV. An APACHE II score of more than 17 ( P = 0.022) and a respiratory rate of more than 25 breaths·min−1 after 1 h of NIV ( P = 0.024) were independent factors associated with the need for endotracheal intubation. Patients who avoided endotracheal intubation had a signifi cantly lower ICU mortality rate and in-hospital mortality rate than patients who required endotracheal intubation.  Conclusion. However, failure of noninvasive ventilation may lead to an increased mortality. Design: Retrospective study to determine the frequency of NIV failure and identify its determinants.Setting: Medical intensive care unit in a University hospital. Patients: All consecutive pts with hematologic malignancies admitted to the intensive care unit over a 10-yr period who received noninvasive ventilation. Results: 99 pts were studied. SAPS II at admission was 49 (39-57). 53 pts (54%) failed noninvasive ventilation and required endotracheal intubation. Their Pao 2 /Fio 2 ratio was significantly lower (175 [101-236] vs. 248 [134-337]) and their RR under NIV was significantly higher (32 br/min [30-36] vs. 28 [27-30]). 47 pts (89%) who failed noninvasive ventilation required vasopressors. Hospital mortality was 79% in those who failed noninvasive ventilation, and 41% in those who succeeded. Patients who failed noninvasive ventilation had a significantly longer intensive care unit stay (13 days [8-23] vs. 5 [2-8]) and a significantly higher rate of intensive care unit-acquired infections (32% compared with 7%). Factors independently associated with noninvasive ventilation failure by multivariate analysis were respiratory rate under noninvasive ventilation, longer delay between admission and noninvasive ventilation first use, need for vasopressors or renal replacement therapy, and acute respiratory distress syndrome. Conclusions: Failure of noninvasive ventilation occurs in half the critically ill hematologic patients and is associated with an increased mortality. Predictors of noninvasive ventilation failure might be used to guide decisions regarding intubation.

86 Cardiovascular function In healthy subjects, nasal CPAP of 15 cm H2O decreased cardiac output 20%–30%. In patients with stable COPD, high (10–20 cm H2O) pressure-support with low (3–5 cm H2O) PEEP decreased cardiac output approximately 20%. In patients with ALI those NIV levels had negligible effects on cardiac output. In patients with congestive heart failure, NIV often increased cardiac output by decreasing inspiratory effort and left-ventricular afterload. Pulmonary gas-exchange function At settings that minimized WOB, NIV typically increased pH an average 0.06, increased PaO2 8 mm Hg, and decreased PaCO2 9 mm Hg. London, Ontario, Canada SEARCH STRATEGY: We searched the Cochrane Central Register of Controlled Trials MEDLINE (Jan 1966 - July 2003) and EMBASE (Jan 1980 - July 2003) for RCT comparing NPPV and IPPV weaning. SELECTION CRITERIA: Randomized and quasi-randomized studies comparing early extubation with immediate application of NPPV to IPPV weaning in intubated adults with respiratory failure. DATA COLLECTION AND ANALYSIS: 2 reviewers independently assessed trial quality and abstracted data according to prespecified criteria. MAIN RESULTS: We identified 11 trials, of which 5 were included, involving 171 participants with predominantly COPD. Overall, the included studies were of moderate to good quality. NPPV strategy decreased mortality (RR 0.41, 95% CI 0.22 to 0.76), the incidence of ventilator associated pneumonia (RR 0.28, 95% CI 0.09 to 0.85), intensive care unit LOS (WMD -6.88 days, 95% CI -12.60 to -1.15), hospital length of stay (WMD -7.33 days, 95%CI -14.05 to -0.61), total duration of mechanical support (WMD -7.33 days, 95% CI -11.45 to -3.22) and the duration of endotracheal mechanical ventilation (WMD -6.79 days, 95% CI -11.70 to -1.87). There was no effect of NPPV on weaning failures or the duration of mechanical support related to weaning and insufficient data to pool adverse events or quality of life. Subgroup analyses suggested that the mortality benefit of the NPPV approach is greater in patients with COPD. REVIEWER'S CONCLUSIONS: Summary estimates from 5 studies of moderate to good quality demonstrated a consistent positive effect on overall mortality. At present, use of NPPV to facilitate weaning in mechanically ventilated patients, with predominantly chronic obstructive lung disease, is associated with promising, although insufficient
