ΒΑL (BronchoAlveolar Lavage) Βρογχοκυψελιδικό έκπλυμα ) BAL evaluates cellular and non-cellular constituents of the bronchoalveolar space. This space is anatomically inhomogeneous and consists of bronchi, bronchioles, and alveoli. BAL is now a standard diagnostic procedure in all patients with unclear diffuse lung abnormalities, whether an infectious, noninfectious immunologic or malignant etiology is suspected. BAL recovers cells and solutes from the lower respiratory tract, and thus serves as a ‘‘window to the lung” is an established diagnostic tool in evaluating inflammatory and immune processes of the lungs Lung parenchyma

Έκκριση Surfactant από κύτταρα τύπου στον κυψελιδικό χώρο Ο επιφανειoδραστικός παράγοντας των πνευμόνων είναι μίγμα λιπιδίων και πρωτεϊνών που επικαλύπτει το εσωτερικό των πνευμόνων. Μειώνει την επιφανειακή τάση στις κυψελίδες και τις προστατεύει από τη σύγκλιση, ιδίως κατά το τέλος της εκπνοής. Επίσης ασκώντας πίεση στο επιθήλιο περιορίζει τη μεταφορά υγρών από τα τριχοειδή αγγεία στο διάμεσο χώρο και στις κυψελίδες

ΒΑL proteome analysis Many proteins identified in BALF are plasma proteins, probably originating from it by passive diffusion due to the permeability of the air-blood barrier. However, among these proteins, some are synthesized in the lung too (i.e. 1-antitrypsin, -e macroglobin, apolipoprotein A1, 2-microglobulin, ceruloplasmin, complement factors 3 and 4, immunoglobulins), probably to provide local extra- protection against invading micro organisms, oxidative damages and proteases released during inflammatory processes taking place in the lung. Other constituents are antioxidant proteins, cytoskeletal proteins, or proteins involved in tissue repair and proliferation, in immunological and inflammation responses and in lipid metabolism.

•BAL •BAL is a diagnostic tool in evaluating inflammatory processes of the lungs. •PLase A 2 could play important role in the development of inflammation in ARDS and could serve as a prognostic factor for the outcome of the patients. •PAF-AcH could contribute to the control of inflammation in ARDS. •In ARDS, the distribution of PLase A 2 in small surfactant aggregates with poor surface function indicates that surfactant phospholipid hydrolysis could be due to this enzyme.
