Slide 2. Incidence of Bone Metastases A wide range of cancer types metastasize to the bone. Breast and prostate cancer are among the most prevalent types of malignancies that are associated with a high incidence of bone metastases.1,2[Ferlay 2001][Coleman 1997 p1589] These cancers are probably responsible for more than 80% of cases of metastatic bone disease.2[Coleman 1997 p1588] Bone disease caused by metastatic breast cancer often manifests as a mixture of osteolytic and osteoblastic lesions,3[Hortobagyi 1991 p11] whereas prostate carcinoma is usually associated with the development of osteoblastic bone lesions.4[Carlin 2000 p2990] Multiple myeloma, although not as prevalent as breast or prostate cancer, also has a high incidence of bone metastases.1,2[Ferlay 2001][Coleman 1997 p1589] Over 87% of patients with newly diagnosed multiple myeloma have evidence of bone involvement.5[Lecouvet 1999 p35] The major clinical consequences of multiple myeloma metastatic to the bone are the development of osteolytic bone lesions, pathologic fractures, osteoporosis, and hypercalcemia.6[Zaidi 2001 p275] References 1.Ferlay J et al. The OPG/RANKL system is required for the final step of osteoclastogenesis, in which excessive new bone (osteoblastic) is deposited around tumour cell deposits, away from osteolytic sites of bone resorption, resulting in low bone strength and potential vertebral collapse 4. Osteoclastic (osteolytic) activity releases growth factors (TGF-beta) that stimulate tumour cell growth, perpetuating a vicious cycle of excessive bone resorption 5. Slide 11. ZOMETA® in Bone Metastases, Phase III Trials The efficacy of ZOMETA was evaluated in 3 randomized, double-blind clinical trials. The efficacy of ZOMETA in patients with breast cancer or multiple myeloma was compared with that of pamidronate, a bisphosphonate previously shown to be effective in reducing SREs in these cancers. ZOMETA® Reduces the Risk of Developing Skeletal Events Unlike the skeletal morbidity rate, the Andersen-Gill analysis considers not only the number of events within a given time frame, but the time between each SRE. This is a more robust means of looking at the data and is more clinically meaningful, as occurrence of 1 SRE increases the risk for future SREs. Thus, delaying the time between SREs is an important benefit of bisphosphonates. In all 3 trials, ZOMETA reduced the risk of developing skeletal events as compared with placebo and pamidronate. Significant reductions were observed with ZOMETA treatment for prostate cancer (36%) and lung/other solid tumors (31%) versus placebo. Similarly, ZOMETA significantly reduced the risk of developing skeletal events by 16% in the breast cancer/multiple myeloma population, with the largest risk reduction occurring in the breast cancer cohort.
